Palliative Care in the Roaring Fork Valley

Caring for a loved one during a serious illness requires both strength and tenderness. Our palliative caregivers provide comfort and support for individuals and families facing the challenges of advanced illness, offering a steady presence when it’s needed most. We understand that meaningful care is about more than physical assistance — it’s about preserving dignity, easing discomfort, and nurturing emotional well-being. With patience and compassion, our caregivers honor each person’s wishes and routines, bringing peace, understanding, and gentle companionship to every moment.

Whether it’s…

assistance with personal care…

facilitating time with family…

or emotional support…

Here to There Palliative Care

Comfort and Compassion — right at home.

Here to There caregivers provide comfort, compassion, dependability, and organization during a difficult time for any family. It’s a time that not only affects the person receiving care but everyone who loves them. It’s a time to bond and reflect and to say goodbye.

Let us help you by taking care of all of the daily details. Our caregivers are experienced in helping families through the process in a very calm and compassionate way. We have worked closely with the wonderful HomeCare and Hospice for years. We are proficient organizers and excel at facilitating communication between everyone involved in your loved ones care and everyone who needs to stay informed.

We know that every situation is unique, so we select a caretaker and tailor a care plan that’s specific to your family’s needs. During your initial consultation, we listen closely to clearly understand what matters most to you and how we can be the most help.

Our caregivers are trained to recognize subtle changes in comfort, energy, or mood — allowing families to respond proactively while feeling supported at every stage of care. Whether your loved one is recovering from treatment, managing chronic illness, or nearing the end of life, our team ensures compassionate, consistent care rooted in dignity and understanding.

Heather, the founder of Here to There, is a Carbondale native who truly cares about seniors in the Roaring Fork Valley. She has helped local families navigate end-of-life care as a family member, a friend, and a caretaker throughout her entire adult life. Now, as the owner of a home health care agency, she instills our caregivers with the same principles that made her such a calm and reassuring presence for so many.
